Bone grafts, graft membranes, Invisalign, Vivera™ retainers, and nitrous oxide are billed at regular fees and are not included in this plan's discounts.

This plan cannot be combined with dental insurance or any other dental plan, and financing options are not available.

This is a discount plan only, honored exclusively between the patient and the office and staff of Dr. Hafid E. Ortega, DDS.

All services and fees must be paid in full at the time of service.

This is an individual plan and cannot be transferred to another person.

If a cleaning appointment is missed without 48 hours' notice, it will count as one of your prepaid cleanings for the year.

Your plan year begins on the date this agreement is signed and is honored for the following 12 consecutive months.

This plan is non-refundable. No refunds will be issued at any time if you choose not to use the plan.

Canceling or no-showing an appointment with less than 48 hours' notice voids this agreement.
By enrolling, you acknowledge that this plan is honored only with Dr. Hafid E. Ortega, DDS and staff, for a 12-month period, and is non-refundable.
We are a full service dental office with over 13 years in the same location! We offer same day crowns and complete care including: implants, fillings, dentures, orthodontics and periodontal services. We specialize in utilizing the latest state of the art dental equipment. We offer full service dentistry for children and adults. For people with existing insurance, your cleanings could cost $0!
